Membership fees start as low as $70 per month based on patient’s age.

Initial visit/enrollment fee: $125/individual - $200/family

What’s Included

Standard:

  • 1 comprehensive one-hour physical exam/well child exam annually with blood sugar and cholesterol testing

  • Lab reviews

  • Review screenings and referrals, if needed

  • Standard office visits at least 30 minutes

  • Chronic disease management, such as diabetes, hypertension, and depression

  • Commercial Drivers License examination

  • Access to discounted:

    • In-office testing and procedures

    • Labs

    • Generic medications

    • Workshops

  • Prompt scheduling for visits (48-72 hours)

  • Extended access via text/email/video calls

Weight Management

  • Integrative health coaching

  • Weekly to biweekly check-ins

  • Medication prescription(s) as appropriate

  • Support creating movement/dietary strategy

  • Movement-focused visits as desired (“walk and talk” visits)

  • Wellness focused workshops & seminars quarterly
