Membership fees start as low as $65 per month based on patient’s age.

Initial visit/enrollment fee: $125/individual - $200/family

What’s Included

Standard:

  • 1 comprehensive one-hour physical exam/well child exam annually

  • Lab reviews

  • Review screenings and referrals, if needed

  • Standard office visits at least 30 minutes

  • Chronic disease management, such as diabetes, hypertension, and depression

  • Commercial Drivers License examination

  • Access to discounted:

    • In-office testing and procedures

    • Labs

    • Generic medications

    • Workshops

  • Prompt scheduling for visits (48-72 hours)

  • Extended access via text/email/video calls

Lifestyle Medicine:

  • Integrative health coaching addressing movement, meals, mind, medicine for management of chronic illness including obesity

  • Weekly to monthly data review (i.e. step counts, blood sugar or blood pressure logs, food logs)

  • Detailed nutritional guidance (as needed)

  • Detailed movement prescription (as needed)

  • Movement-focused visits (“walk and talk” visits)

  • Wellness focused workshops & seminars
